  • Employer with one or more employees that have individually owned health insurance plans.
  • Employee cannot be eligible for group sponsored health plan.
  • Used to pay for individually owned health insurance premiums with pre-tax dollars from an employee’s paycheck – no employer funding.
  • Not available in all states.
